Community Home Based Care (CHBC) Officer Baylor

BACKGROUND: The Baylor College of Medicine Children’s Foundation – Uganda (Baylor -Uganda) headquartered at Mulago hospital, is a comprehensive HIV care and treatment organization that supports the treatment and care of HIV infected and affected Children and Adults. Baylor – Uganda has three operational areas; HIV/AIDS prevention, care and treatment services, Health Professional training and Clinical Research.

Baylor-Uganda is in receipt of PEPFAR grant through CDC, and is looking for knowledgeable, enthusiastic and compassionate individuals to become part of its comprehensive care and treatment team.

Community Home Based Care (CHBC) Officer (2 positions)

Role definition: Review and implement guidelines for providing community home based care services for comprehensive HIV/AIDS services in Baylor Uganda health facilities and communities in line with the directorate mission

Work station: Head Office

Key result Areas:

1. 1. Develop and implement plans and budgets for CHBC activities in the respective attached zones and regions in line with the CHBC score card and Baylor Uganda finance guidelines
2. 2. Build capacity of VHTs and other relevant community stakeholders, in providing comprehensive CHBC in line with CHBC mission and MoH guidelines
3. 3. Review and monitor the implementation of CHBC activities as per the set agreed upon indicators in line with the CHBC Mission
4. 4. Conduct annual appraisals for VHTs against the agreed upon and set targets in line with CHBC mission
5. 5. Conduct Home visits for difficult and problematic clients to ensure adherence to Care and to provide psychosocial support in line with CHBC mission
6. 6. Review and monitor the implementation of a strategy to mobilize VHTs and health workers for provision of comprehensive CHBC activities in Kampala zone and other Baylor supported regions in line with Baylor Uganda CHBC mission and MoH CHBC guidelines
7. 7. Providing HBHCT (home based HIV counselling and testing) to households of index clients in line with CHBC mission
8. 8. Prepare and submit the quarterly reports to CHBC supervisor in regard to progress of HBC program implementation in the respective zones and attached regions
9. 9. Conduct mapping of Baylor clients homes for provision of HBC (home based care) in liaison with VHTs in respective zones according CHBC mission
10. 10. Carry out research and present researched psychosocial and community home based care best practices in CMEs, conferences and other medical related foras in line with Baylor Uganda research and MoH guidelines

Job specifications:

The job holder should have;

1) A Bachelor’s (honors) degree in Adult & Community education or Social Work and Social Administration or Community Psychology or Developmental Studies

2) at least 3 years experience in implementing Home Based Care Programs; with a reputable international NGO, and direct experience with government and donor representatives

3) have Key personal competences in Team and communication skills, networking and partnership skills. Must be dynamic, Persuasive &convincing, self- motivated and able to demonstrate high initiative.
Salary: - USD/month
