Interview older adult clients to determine problems they encounter, such as personal and family adjustments, finances, physical and mental impairment, to determine nature and degree of problem.
- Provide client accurate and appropriate information on services, benefits and entitlements.
- Develop case notes, action items and follow-up to determine action outcome.
- Consult with program directors to ensure that client needs are met that reflect culturally sensitive options that make efficient use of community resources.
- Advocate and intervene with other departments or agencies on behalf of the participants.
- Act as liaison with other agency programs and external resources.
- Conduct on-site visits with new program participants to assist with engagement with other program members.
Qualifications: Must have a Bachelor's Degree in Human Services or in a related field. Experience working with older adults preferred. Knowledge of benefits/entitlements a +.Must be bilingual in English/Korean)
