Under the supervision of the Director, the Case Manager is responsible for maintaining a case load of at least 22 to 25 clients. Case Management consists of assessing the client's needs and barriers to housing. The main goals of a Case Manager are to help the clients obtain better housing, increase income, and access treatment resources.
Responsibilities:
- Maintain a case load of at least 22 to 25 clients. Develop professional relationship with the client by engaging and building rapport.
- Work at least one evening per week.
- Conduct psycho-social assessments of new clients and clients who are presenting with severe mental health symptoms, history of substance dependence, and homelessness. The psycho-socials are completed using Montgomery County's Assessment Tool.
- Based on assessments provide appropriate intervention and support, especially if the client has emergency needs.
- Based on assessment, devise an individualized service plan in conjunction with the client. The Service Plan must address goals related to obtaining better housing, increasing income, treatment, and a daily schedule.
- Meet with clients once a week to determine the client's progress, determine effectiveness of interventions, and to ensure the client is involved in all levels of Case Management. Devise and maintain clinical progress notes in a timely manner.
- Evaluate client's needs and collaborate with appropriate agencies to ensure client's needs are met. Refer clients to appropriate resources, agencies and housing programs.
- Develop behavioral contracts with clients as needed.
- Provide crisis intervention for clients experiencing acute psychiatric symptoms, engaged in conflicts with other clients, and/or intoxicated.
- Conduct assessments for suicidal and homicidal ideation, intent, and risk. Provide crisis intervention for clients who are suicidal or homicidal by calling the Mobile Crisis Team or the police.
- Refer clients to psychiatric hospitals, inpatient programs, substance abuse treatment programs, the Crisis Center. At times, accompany clients to interviews at mental health treatment programs, housing programs, and vocational programs.
- Participate in interdisciplinary treatment team meetings to discuss appropriate intervention, make case presentations, and program announcements.
- Maintain Homeless Management Information System (Service Point) weekly. The full time Case Manager must maintain the entry/exit Service Point daily, add Service Transactions, and complete the Entry/Exit for each client.
- Track Service Fees paid for by the clients, submit service fees to accounting monthly.
- Other duties as assigned.
Qualifications:
- Minimum of a Bachelor Degree in behavioral health with significant experience working with the target population or a master's degree in Social Work, Counseling, or Psychology.
- Proficient in Spanish and English.
- Knowledge of principles and techniques of mental health assessments.
- Knowledge of signs and symptoms of mental illness, substance abuse, family dynamics, suicidal and homicidal ideation, and developmental stages.
- Ability to work at least one evening.
- Ability to work with a team and take initiative.
- Must work a minimum of 40 hours per week.
- Excellent written and oral communication skills.
- Knowledge of crisis interventions.
- Proficient with varying computer software. Able to use computer with minimal support.
Supervision:
The Case Manager reports to the Program Director.
