Salary Schedule BU 3: $ 26,775 (minimum 2012)
The mission of Heartland International Health Center (HIHC) is to improve the well-being of the communities we serve by providing accessible, high-quality health care. Last year, our nine locations on the North Side of Chicago provided more than 13,000 patients with a medical home where they could receive comprehensive services including primary care, prenatal care, pediatric care, immunizations, women’s health care, chronic disease management, medication discounts, oral health, mental health, and psychiatric services.
Summary: The Benefits and Entitlements Specialist provides services related to securing benefits for patients including eligibility screening, completing applications, and tracking progress of applications. The Benefits and Entitlements Specialist represents Heartland International Health Center to external organizations and develops an astute awareness of public assistance programs. Individual accountabilities and work volume will be established through the development of annual Success Objectives, within the framework outlined below.
Essential Duties and Responsibilities:
·Initiates procedures for patient’s entry into the health care delivery system
·Assists the potential and current patient(s) with the preparation, processing, and completion of insurance (state and private) forms and other documents as indicated
·Helps identify and determine patient(s) payment sources and community resources available to defray medical cost
·Assists in the maintenance of appropriate reports for funders, insurance groups and governmental agencies
·Actively communicates Heartland services to individuals and groups to aide in assessing care for participants
·Provides on-going information/reports to the Director as requested
·Handles correspondence/telephone calls/personal interactions professionally and with the provision of appropriate information
·Participates in the development and revision of written procedures as needed
·Increases community awareness of health services by assisting with functions at community events, schools, churches, and other appropriate organizations.
·Explains and interprets policies, procedures and services to program participants as needed
·Assesses existing patients who are currently classified as Self-Pay and reviews their eligibility for public benefits
·Screens and counsels patients on a one-to-one basis regarding public benefits eligibility
·Assists eligible patients in completing appropriate benefits applications
·Serves as a facilitator by interfacing with the Illinois Department of Public Aid and other public agencies on behalf of patients.
·Provides training to program staff related to benefits and entitlement issues
·Advises medical Staff and/ or other staff regarding documentation needed to obtaining benefits·
·Performs other related duties as assigned/required.
·Other duties may be assigned.

·Education and/or Experience: High School diploma and three years of community outreach or demonstrated work experience with community groups, community health care experience in with minority populations; or a Bachelor's Degree and 1 year of community outreach and community health care experience with minority populations
·Language requirements: Bilingual in Spanish and English may be required
