AHF is seeking a District Area Manager to work remotely/from a Home Office, on a Full-time, Regular basis. We offer a competitive salary, amazing benefits, and a great work environment.

Summary
The District Area Manager provides oversight of the care management staff and other staff and is responsible for the establishment and achievement of goals and objectives for the care management department. For Disease Management assignments, the District Area Manager will assist the Regional Manager in the achievement and maintenance of department and company goals and objectives outlined in the HIV/AIDS Disease Management Program. 

Essential Duties & Responsibilities
Includes the following. Other duties may be assigned.

• Assist in the development and implementation of care management for the HIV/AIDS Disease Management Program.

• Assist in the development, implementation, achievement and periodic evaluation of the Disease Management Department goals and objectives related to utilization management, quality improvement, productivity, and overall departmental goal achievement.

• Manage daily and overall regional needs to assure compliance with expectations of departmental, regional, company, contractual and governmental goals and regulations.

• Evaluate and recommend continuous improvement to programs operating in his or her geography on an ongoing basis and participate in the annual evaluation process.

• Oversees care management activities of the nursing and support staff within an region, including, but not limited to, enrollment and disenrollment processes, initial assessments completion, care plan development, implementation and modification, acuity determination, claims assessment, member reassessment, utilization monitoring, provider visits by care managers, ASO collaboration by care managers, discharge planning, member education and adherence monitoring.

• Conducts orientation meetings with providers, hospitals, ASOs and other community members concerning disease management, the purpose and benefits of the program, processes employed to track and manage the HIV/AIDS member population. 

• Develops and maintains proficiency in PosiTrak. Assures all documentation is up do date. 

• Monitors PosiTrak documentation frequency, content, quality, and overall adherence to evidence based standards of care.

• Maintains NCQA standards at all times. 

• Reviews PosiTrak documentation by staff on an ongoing basis. Utilizes reviews for staff development, process improvement, and monitoring of productivity. Addresses documentation issues with each individual staff member as necessary.

• Conducts monthly one-to-one conference with Care Manager and other appropriate staff to review roster of member’s assessment/reassessment, member and overall roster acuity, interventions, successes, barriers, provider visits, chart review, community outreach, attendance at community meetings and achievement of goals. In collaboration with care manager sets and documents goals for the coming month. 

• Assures the care managers participate in the on-call coverage to ensure 24 hour nurse advice availability to program members. .

• In collaboration with HR assures licensed staff maintains current licensure. 

• Utilizes and maintains compliance to HR management documentation requirements, progressive discipline, evaluations, career development tools, and other items as required. Assures documentation is sent to HR in a timely manner

• Provides, on a monthly basis, all necessary information, documentation and monthly written summary reports required to the Regional Manager for inclusion in the Program monthly, quarterly, and annual reports required by the contract and AHCA. 

• Collaborate on activities with other Regional Care Managers, Director, Bureau Chief and/or the Chief of Managed Care.

• Participates in the Continuous Quality Improvement Program. Serve on committees when requested.

• Plan, coordinate, implement and document the education, orientation and training of Regional personnel.

• Work closely with the Director and the Medical Director toward the overall achievement of improved outcomes and accessible, cost effective, and quality health care for the members of the region.

• Encourage and participate in growth and development of AHF (all business lines) that operate within his or her geography. 

• Oversee and assure that staff and programs are in compliance with contract requirements for the DM program operating within his or her geography. 

• Will maintain a caseload of a maximum of 15 active clients.

Participation in AHF Meetings/Committees
• Attends AHF meetings as assigned.

Supervisory Responsibilities
Carries out supervisory responsibilities in accordance with AHF’s policies and applicable laws. Responsibilities include interviewing, hiring, and training employees; planning, assigning and directing work, appraising performance; rewarding and disciplining employees; addressing complaints and resolving problems. Directly supervises employees in the Care Management Region (care managers and administrative staff), and provides supervisory guidance to employees of other business lines operating within the geography including but not limited to Testing Operations, Advocacy Operations and other operations as assigned.

Education and/or Experience
Bachelor’s degree (B.A.) from four-year college or university; or one to two years related experience and/or training or equivalent combination of education and experience.

Computer/Software Skills & Abilities
To perform this job successfully an individual should have knowledge of MS Office Word processing software, Excel spreadsheet software, and Contact Management software.

Language Skills
Ability to read, analyze, and interpret common scientific and technical journals, financial reports, and legal documents. Ability to respond to common inquiries or complaints from customers, regulatory agencies, or members of the business community. Ability to effectively present information to top management, public groups, and/or boards of directors.

Mathematical Skills
Basic Skills: Ability to add, subtract, multiply, and divide in all units of measure, using whole numbers, common fractions, and decimals. Ability to compute rate, ratio, and percent to draw and interpret bar graphs.

Reasoning Ability
Very High Skills: Ability to define problems, collect data, establish facts, and draw valid conclusions. Ability to interpret an extensive variety of technical instructions in mathematical or diagram form and deal with several abstract and concrete variables.

Other Skills and Abilities
Required:

• One (1) year of experience in the care of HIV/AIDS patients.
• One (1) year of experience in Medicaid or Community Service entities that are responsible to the HIV/AIDS patients.
• Demonstrated technical and clinical skills related to the care of HIV/AIDS patients.
• Strong decision-making skills.
• Ability to function well in a fast-paced and changing work environment.
• Knowledge of principles of total quality management and continuous quality improvement.
• Excellent organizational skills.
• Ability to travel up to 50% of the time.

Preferred:
Certification as ACRN and Case Management.

Certificates, Licenses and Registrations
Registered Nurse license with the State of Florida
Valid driver’s license and proof of automobile liability insurance.
